The Journey Begins…

My name is Lynne Nohara-Kido, DPT and for the past 16 years, I worked at the Rehabilitation of the Pacific as an inpatient Physical Therapist, in charge of the Neuro Service Line and specializing in brain injury and stroke recovery.

In February of 2023, my father sustained a fall and a resulting traumatic brain injury. Since my experience thus far had been in brain injury recovery, I felt confident in my ability to assist my parents in his transfer home and recovery. Until this event, my only experience had been as a medical practitioner, providing training to patients and their families prior to discharge. I had not experienced the process of taking someone home from the hospital, entering the home and settling them into a routine. Fortunately, my previous experience equipped me to be able to negotiate the details and problem solve unforeseen issues that arose after my father’s discharge home. I began to wonder how others would fare in similar situations and without the resources and experience that I had developed over the years.

With over 16 years of inpatient rehabilitation experience as a Physical Therapist, I have assisted countless families and patients prepare to return home. As a part of that journey, my responsibilities have included home assessments, visits, equipment ordering, caregiver training, etc. with the goal of returning patients to their home or the safest discharge location possible.  However, once that individual left our facility with their family, our connection stopped. With Kauhale Smooth Transitions, a Limited Liability Corporation (LLC), the hope and goal is to bridge the gap in the continuum of care from facility to home.
