Feel the support and reassurance of a smooth transition back to your familiar surroundings with “hands-on” training during your transition home.
Intermediate Service (Client is present)
May Include:
Basic Assessment services
Post-discharge hands-on training: Transfer training, Elevation Training, Equipment Training, etc
Overall assessment of client mobility within the home (Daily schedule development, Home exercise programs, etc)
Review of discharge instructions (if discharge home from a facility)
